• In accordance with Health Insurance Portability and Accountability Act of 1996 (HIPAA) I hereby authorize Easicient to review any and all health information related to my records shared with Easicient. I understand that the information in my health record may include information related to immunodeficiency virus (HIV). It may also include information about behavioral or mental health services, and/or treatment for alcohol and/or drug abuse.
